DMCA POLICY
At Health Wellbeing Centre, we value and respect the intellectual property rights of others, just as we expect others to respect our rights. In accordance with the Digital Millennium Copyright Act (DMCA), Title 17, United States Code, Section 512(c), copyright owners or their agents may submit a takedown notice to us via our designated DMCA Agent listed below. As an internet service provider, we claim immunity from infringement claims under the "safe harbor" provisions of the DMCA.
Submitting a Takedown Notice
To submit a good faith infringement claim, please provide us with a notice containing the following information:
Notice of Infringement – Claim
1. A physical or electronic signature of the copyright owner (or an authorized agent);
2. Identification of the copyrighted work claimed to have been infringed;
3. Identification of the infringing material, along with the URL of the page containing the material;
4. Contact information for the complaining party, including name, physical address, email address, phone number, and fax number;
5. A statement by the complaining party that they have a good faith belief that the use of the material is unauthorized; and
6. A statement that the information in the notification is accurate, and, under penalty of perjury, that the complaining party is authorized to act on behalf of the copyright owner.
Under Title 17 USC §512(f), penalties may apply to any person who knowingly and materially misrepresents information in a notification of infringement.

Counter Notification – Restoration of Material
If you have received a takedown notice and believe it was in error, you may submit a counter notification to have the material restored to the site. Your counter notification must contain the following elements pursuant to 17 USC Section 512(g)(3):
1. Your physical or electronic signature;
2. Description of the material that was taken down and its original location;
3. A statement under penalty of perjury that you believe the removal was a mistake or misidentification;
4. Your name, address, and telephone number; and
5. Consent to jurisdiction for legal proceedings.

Repeat Infringer Policy
We maintain a list of DMCA notices and make a good faith effort to identify repeat infringers. Accounts found to violate our repeat infringer policy will be terminated.
Modifications
This DMCA policy may be modified at any time without prior notice. Please review this policy periodically for any changes.
